PRISM Holiday RegistrationNovember 08, 2020

Every holiday season, City of Life holds an outreach called Angel Tree, where people from City of Life purchase Christmas gifts for children (under age 18) of single moms.  

It is our honor as a church to be able to help single moms through the holidays!

Due to the pandemic, this is the first year we are opening up registration online, so there are a few important things to know before registering:

• We ask that single moms register for themselves.  Please do not register on another person's behalf. 
• This program is only for single moms with children ages 18 and under.
• If you have participated in our PRISM program in the past, you have been assigned a unique registration number.  This number is necessary in order to register online and may not be shared. You will need this number to fill out the online form. This number is unique to you and may not be shared among registrants. All former PRISM registrants were sent an email with this number (if you can't find it, check your spam & promotions folders).     If you have NOT participated in PRISM the past and do not have an ID number, or you have participated and cannot locate your ID number in the email, please contact Amy Cintron at amy.cruz@col.tv

When filling out the form, be sure you click the "Submit" button at the very end so that your registration goes through.
